Eyelid cross section, excision for basal cell carcinoma on the lid margin. (The EALT is not that impressive under the conjunctival epithelium in this section but the glands came out better! 🤷‍♂️)

Interestingly I have not seen as many LD related to immune check point inhibitors, but rather seen more BP lol.. I guess LD was treated without biopsy. ??

TWO minutes! If you “OOPS!” place your DIF bx in formalin, remove it WITHIN 2 minutes to have a decent chance to salvage a +DIF and avert a false negative result. Ideally even faster if seeking pemphigus.

LeBoit: NTRK1-fused spitzoid neoplasms show filigree-like rete, lobulated nests, confluent large nests with large cells (photo) with hypermaturation (cells gets much smaller with descent). Can be atypical but extranodal spread rare. IHC available.

LeBoit: ALK-fused Spitz have specific morphologic features that can be helpful. High mitotic rates and large nests superficially but maturation present. Caution in superficial biopsies for this reason. Anecdotally, these do not show extranodal spread.
