not quite age, sex = 0.73 age, sex, genetics = 0.76 PCE (which includes age, sex, biomarkers at middle age) = 0.76 so biomarkers at middle age or genetics add the same (about 0.03) on top of age and sex

Yes For heart attack risk prediction in population, age & gender trump all (ie the pooled cohorts equation in US is essentially a way to identity old men) Principal advantage genetics: “30-year head start” on identifying those at relatively higher risk

😳 A model with three variables available at birth (brithdate, gender, polygenic score) equal in heart attack risk prediction to a bunch of variables measured in middle age

Here are the c-stats for 4 models of CAD risk prediction in 350K UK Biobank pts published in JAMA: Age+Sex=0.73 Age+Sex+Genetics=0.76 Age+Sex+Race+SBP+HTN Tx+TC+HDL+DM+Smoking (pooled cohort equation) = 0.76 Pooled Cohort Eq+Genetics = 0.78 In 5 yrs, which will we be using?

Thx Some logistical challenges (solutions): 1. Need genotyping data (25M in US have this already; UK creating 5M cohort) 2. Screen 5 to get 1 eligible (leverage those with existing data) 3. Cost (one-time blood test, <$20, same data useful for other diseases)

Want to double the event rate in CVOT --> recruit those in higher end of polygenic score distribution CAD polygenic score has been evaluated in 5 secondary prevention CVOT (CARE, PROVE-IT, FOURIER, ODYSSEY-OUTCOMES, ACCELRATE) A remarkably consistent finding
