I’d say yes to a transplant from an HIV+ organ tomorrow, Cam and Ghady! I was thinking more with the regulatory and clinical environment—what the system would tolerate re: donor infection, recipient risk w/o cure. For me, managing HIV > long, risky wait for transplant!

We definitely have more work to do to maximize organs for transplant that have an increased risk of transmissible disease. We have many recovered IRD organs that are not transplanted in the US—they are discarded instead.

The EXPANDER and Penn THINKER studies show the tremendous potential of HCV donor+ (D+)/recipient-(R-) transplants. I think HIV D+/R- is a very long way off, since we don’t have the equivalent of DAAs for HIV (yet).

There is no debate! PHS high-risk organs of good quality function pose very low (acceptable) risk of infection transmission and should be used in pediatric recipients. Great discussion by many pediatric transplant thought leaders!
