thats what its is about. showing what the hell is the actual problem, even if its complicated. its not something that can be read and understood within 5 minutes. it has to be studied and hopefully will point out all the weaknesses of a system that lead to the accident.

but as a our systems and organisations grew more complex, with more complicated technologies, more entities involved, these models showed to be inadequate to actually show what the hell was the actual problem, so to speak.

u probably know other visualisation techniques for accidents, like the risk matrix or swiss cheese model. they have been developed in a time when systems where less complex and the understanding of safety did not include social/organisational factors.

the goal is to visualize complexity. just like a programmer would do a graphic representation of his/her program to better understand what the hell it is actually doing later, when changes have to be made to it, for example.

yes, thats a legitimate question. ok, let me try to explain. first, system engineers like to put everything into graphs. not necessarily to make it easy to grasp with one look, but in a way that it reflects actual reality and shows things that are not obvious in the first place.
