They are very disappointing statistics. I've seen too many "huddles" outside curtains before the fabric is breached and the decision announced after " How are we today?" What does that even mean? How do I know how the doctor is feeling or what relevance it has to my condition!

Sad but all too familiar example of care that isn't person centred. Almost half (44%) of NHS inpatients say they wanted to be more involved in decisions about their care; one in nine don't feel involved at all.

I know that and I have been presenting at conferences and delivering workshops on pts right to have the information they need to manage their health & keep themselves safe. It feels very different when I am lying in a bed & people are making decisions about me dep on my approach.

I understand that the "professionals" have a lot of experience and expertise to offer. I also have a lot of experience and expertise to offer. The safest and most effective plan is when the two come together! I can understand how pts feel excluded when plans are just announced!

Having said that, there are lots of doctors, nurses and other healthcare professionals who really want to share information and work things out together. It's just that there is an enormous gulf in attitude about most things that are important to patients and keeping them safe

Ok...in hosp in bed in cubicle. Usual " gaggle" of cons, reg, other docs just outside my door discussing my care plan. I asked if I could be part of the discussion. The consultant came in & said "we don't do that" then he came in & announced my care plan...oh dear. Just saying

We have created the only existing detailed longitudinal record for my son with complex needs. Every MDT, op note etc is included using Evergreen Life Docs file. There is a search facility where you can search 4docs by type date condition etc This is the only way to keep safe

I wouldnt call it crap at all, I would say it does a stunning job, by it's people, even considering the huge issues and political liabilities / decisions it has to work within, that said it does need to be brave and sort its problems, to make it fit for the next 50 years
