Pregnant woman served cleaning solution instead of latte at McDonald's

McDonald's is saying sorry after a 32-week pregnant mother was served a cleaning agent in a cup rather than a latte in Alberta. The two cleaning lines were accidentally hooked up to the latte machine while her drink was made.

This just speaks to how important attention to detail and safety in the food service industry is...better training and checklists? Or is this a matter of needed required “double confirmation” before the machine can be turned on to minimize human error?
