Did some work with few years ago on poss indicators of quality and falls was on list. As you say, tricky one because home with no falls might be also a concern. Much better indicator is pressure ulcers but reporting of these is also inconsistent.

Took mother from to dentist to replace lost teeth ... Home says she takes her teeth out because of dementia ... Dentist said she had two mouth ulcers ... Bet you would take your teeth out if you had ulcers
