A Starry Night... 78 yo male farmer from IA with 2 week hx of FUO, drenching night sweats and elevated AST/ALT. This is his C. burnetii Phase II IgG IFA...endpoint titer 1:131,072. Phase I IgG was <1:64. Dx: Acute Q-fever!

Leptospirosis (among other IDs) is a post-Florence flooding concern. Cx and PCR are not routinely available...but serology is! Enter the L. biflexa IgM dot blot. Sensitivity: ~85% at 6-10 days and >95% at >10 days post-symptom onset.
