This is what happens when the MR safety culture becomes lax. Note the almost-90 degree bend in the IV pole and in one of the feet. The risk of impalement, crushing, etc. is very real.

This isn’t a freak accident as claims in its headline. It’s failure of basic protocol. And, , an MRI machine is *ALWAYS ON* and has an extremely powerful magnetic field, thousands of times stronger than the earth’s.

Agree👍🏻Time is a barrier, along with increasing operational demand, which causes tension with many aspects of clinical quality🙅🏼‍♂️ We need to somehow make time for Can we better use tech to support reporting and make it easier? Are there other ways we can do this? 🤷🏼‍♂️🤷🏼‍♂️
