This ended up being a 4.5 mm vessel. used as this was only one small part of a larger percutaneous revasc with EF 15% The extra effort upfront will pay dividend in the future for this patient.

Single access today for distal left main Cx CSI atherectomy in pre- patient with EF 30% and AVA 0.6. I have not needed BAV to advance impella across severe AS yet..does need a push, maybe others BAV always?
