Late post w pt permission. PERT activation this week. 64 yo female presenting w CP/SOB for 4 hours. Intubated for RF in ED. SaO2 88% on 100%FiO2. BP initially stable. PASP 51 initially. BP went to 80/60. Inari flowtriever. Sat 100% on table, PASP 21. Extubated 6 hours later.

Possibly the most difficult part of thrombectomy for acute PE is knowing when to stop. Do you use hemodynamics? Oxygen sats? Angiography? Amount of clot? Blood loss? This case today a good example.

25 F 26 weeks pregnant, b/l massive PE. Hypotensive, HR150, PA pressures 90s and severe RV dilatation. S/P Penumbra and left EKOS for 18 hours -> PA pressures and echo normalized 3 days later. Went home on lovenox.
