GA. Total cases: 18157, Recoveries: 31, Active cases: 17445. GA Public Health commissioner sent a letter to all MDs Mar 22 emphasizing no evidence for use of hydroxychloroquine for COVID-19. GA requires prescription to be consistent with evidence of use.

TX. Total cases: 19317, Recoveries: 4806, Active cases: 14012. TX requires a dx & 14 d limit for hydroxychloroquine rx. Since Mar 30 TX DSHS has been shipping HCQ to hospitals & pharmacies throughout the state.

CT. Total cases: 17962, Recoveries: 65, Active cases: 16770. CT strongly advises against the off-label use and prescription of hydroxychloroquine to COVID-19 outpatients and allows for the treatment of inpatients only where clinical trials are unavailable.

MD. Total cases: 12830, Recoveries: 914, Active cases: 11430. Maryland Board of Pharmacists and Board of Physicians ask professionals to exercise judgement in determining the clinical appropriateness of hydroxychloroquine prescriptions, so no major restrictions.

WA. Total cases: 11790, Recoveries: 1778, Active cases: 9378. WA does not restrict hydroxychloroquine for COVID-19 other than to discourage prophylaxis use due to supply & urges providers to "act with their best discretion." On Mar 21 UW announced it would treat inpatients w HCQ.

OH. Total cases: 11602, Recoveries: 120, Active cases: 11011. Ohio allows hydroxychloroquine prescriptions for coronavirus with a positive test and 14 d limit for out/inpatients. However added scrutiny on MD's for self-prescription & hoarding may be dampening treatment efforts.

IN. Total cases: 11210, Recoveries: 14, Active cases: 10634. Indiana allows for a limited supply prescription of hydroxychloroquine & azithromycin with a presumptive diagnosis. However its Health Commissioner has downplayed HCQ as a treatment or preventative for COVID-19.

CO. Total cases: 10447, Recoveries: 559, Active cases: 9402. Colorado allows for a 30 d limited supply of hydroxychloroquine and stipulates that it should be for those "ill enough to be hospitalized for COVID-19."

VA. Total cases: 9630, Recoveries: 1324, Active cases: 7982. Virginia restricts hydroxychloroquine for outpatients, requiring a diagnosis "consistent with the evidence for its use." Inpatient use is allowed, plus "where there is not an alternative therapy."

NC. Total cases: 7113, Recoveries: 883, Active cases: 5989. (Apr 21) North Carolina emphasizes the rights of pharmacists to "refuse to fill prescriptions...that are not clinically appropriate." On Mar 28 DHHS gave an NC pharma co a grant for a 1 mth study on hydroxychloroquine.

MO. Total cases: 5941, Recoveries: 547, Active cases: 5174. (Apr 21) Missouri allows for limited supply rx of hydroxychloroquine for presumptive + COVID-19 patients and for prophylaxis. Recoveries began increasing Apr 5, ~1 wk after FDA EUA. Slow but increasing by the day.

RI. Total cases: 5500, Recoveries: 293, Active cases: 5036. Rhode Island basically asks providers to submit an entire application if they want to invoke off label use of hydroxychloroquine, rendering it all but unworthy of the time and likely persecution for even trying.

AL. Total cases: 5327, Recoveries: 20, Active cases: 5121. Alabama does not restrict the prescription of hydroxychloroquine for off-label use but warns a provider is liable if a patient experiences major problems. 20 recoveries reported Apr 5 1 week after FDA EUA.

AZ. Total cases: 5251, Recoveries: 70, Active cases: 4973. Arizona allows for inpatient/outpatient rx of hydroxychloroquine with a positive diagnosis and limits rx to 14 d. Prophylaxis prohibited. Recoveries increasing slowly from Apr 5. (recall this was fish tank cleaner state)

NV. Total cases: 3937, Recoveries: 1841, Active cases: 1933. Nevada allows hospitals to treat both inpatients and outpatients with hydroxychloroquine. This was the result of an agreement reached bw hospital ER MDs & the governor. NV active cases decreased from 2503 to 1933 today.

IA. Total cases: 3748, Recoveries: 1428, Active cases: 2230. Iowa issued a stmt on hydroxychloroquine, chloroquine, and azithromycin on Mar 26, allowing prescriptions for inpatients/outpatients with a diagnosis. Since Mar 26 (when recov= 0), recoveries have steadily increased.

UT. Total cases: 3445, Recoveries: 888, Active cases: 2523. Utah recoveries began to be reported Apr 5 1 week after FDA EUA. On Apr 20 UT announced it is negotiating to acquire a supply of hydroxychloroquine for 200k pts to be avail free w a prescription and a positive diagnosis.

KY. Total cases: 3373, Recoveries: 1122, Active cases: 2066. Kentucky allows for a limited 10 d prescription of hydroxychloroquine for new patients that were not on the drug prior to the rule eff date. Since the Mar 28 FDA EUA recoveries have increased from 2 recoveries to 1122.

DC. Total cases: 3206, Recoveries: 645, Active cases: 2434. DC allows for a limited 14 d prescription of hydroxychloroquine (10 d azithromycin) with a positive test result. DC began reporting recoveries Mar 28, the day of the FDA EUA and recoveries have been steadily increasing.

OK. Total cases: 2894, Recoveries: 1772, Active cases: 952. Oklahoma allows both inpatient & outpatient use of hydroxychloroquine with a written diagnosis limited to a 14 d supply other than for chronic conditions. On Apr 3 it was announced that OK had secured its own HCQ supply.

NE. Total cases: 3358, Recoveries: 22, Active cases: 3280. NE requires a confirmed diagnosis for all antiviral treatment. Remdesivir is preferred which as of week of Apr 13 had just completed enrollment. Hydroxychloroquine is 3rd choice after plasma & requires ID consultation.

AR. Total cases: 3069, Recoveries: 1965, Active cases: 1053. on Apr 8, Arkansas announced the donation of 100,000 pills of hydroxychloroquine from NJ's Amneal Pharmaceuticals to be used to ensure adequate supply for chronic conditions and some coronavirus pts.

NM. Total cases: 2823, Recoveries: 650, Active cases: 2069. New Mexico asks prescribers and pharmacists to exercise professional judgment and will not "take action against a pharmacist or pharmacy that declines to fill a questionable prescription" for antivirals.

SD. Total cases: 2245, Recoveries: 1316, Active cases: 918. On Apr 13 South Dakota became the first state to implement a statewide trial to treat coronavirus in/outpatients, and provide prophylaxis for healthcare workers & high risk popns. SD's active cases continue to decrease.

NH. Total cases: 1938, Recoveries: 798, Active cases: 1080. NH allows for inpatient and outpatient prescriptions of hydroxychloroquine for coronavirus, with a documented diagnosis. It sets a limit of 30d for new patients, ie those not obtaining refills for chronic conditions.

ID. Total cases: 1952, Recoveries: 1087, Active cases: 805. On Mar 13 Idaho restricted supply to prevent shortages & hoarding but allowed flexibility for change. After FDA sent a shipment of hydroxychloroquine to multiple states on Apr 6, ID began to report recoveries on Apr 13.

WV. Total cases: 1095, Recoveries: 481, Active cases: 576. On Mar 24, WV Governor Jim Justice announced that Mylan's WV facility restarted the manufacture hydroxychloroquine for 1.5 million patients by mid-April. On Apr 10, WV began to report its first 57 recoveries, now at 481.

ME. Total cases: 1040, Recoveries: 585, Active cases: 404. Maine does not restrict outpatient or inpatient rx of hydroxychloroquine, chloroquine, and azithromycin for patients with symptoms/diagnosis but prohibits prophlactic use and prescribing for self, family and friends.

HI. Total cases: 609, Recoveries: 505, Active cases: 88. Hawaii does not restrict the use of hydroxychloroquine for inpatients or outpatients. Hawaii's first recoveries were reported on March 31 after the Mar 28 FDA emergency authorization for HCQ use. Active cases are falling.

MN. Total cases: 4181, Recoveries: 1912, Active cases: 1968. Minnesota's Governor allows the use of hydroxychloroquine & chloroquine for "a diagnosis appropriate for the use" w 30 d limit. UMinn is conducting a nationwide study on HCQ for 1500 symptomatic & asymptomatic patients.
