Ah got it. The aneurysm is small right? The structure anterior to the calcium in the CT is the proximal PA, and not the aneurysm, right? (I obviously don’t read CTs for a living).

Very tempting for retrograde approach. Also although mid vessel is atretic, would plump up w CABG or PCI. But no symptoms and normal EF! I would get him/her on a treadmill to truly determine symptoms. Another question- should you just stop here given lack of ischemia?

Diagnosed my first outpatient w via televisit. Elderly, HTN, new onset HFpEF late-Feb. Started diuretics but dyspnea worsened, then ageusia, aches over 4w, dyspnea plateaued 3d before seeing me. Had been self-quarantining. How would u manage pt? ⁦

Agreed. For patients in LVAD and transplant zone, close f/u is necessary after clip given 29% 2y mortality with clip and 46% with medical therapy alone. You could also argue that at 350mL LVEDV, a disproportionate amount of MR would be huge (ERO 0.50-0.60). Hers was 0.35.
