I would be interested to know which surgical treatment you would recommend that would take 1 hour for a 16 x 18 cm rectal polyp with an endosocpic diagnosis of TVA with high grade dysplasia?

1% recurrence for benign disease post ESD in our practice.

For lesions that are LST G MN with benign pit, we would offer ESD. Decision on further treatment based on histology. If V pit pattern or IIc component with suspected deep submucosal invasion, full thickness with TEMS after appropriate patient counselling.

From someone who does TEMS/TAMIS/ESD and Lap surgery - the key is endoscopic diagnosis prior to selection of procedure. ESD good choice for this case - will share path in due course. Likely TVA with high grade dysplasia. 1% recurrence with ESD.

Endoscopic diagnosis is the key prior to resection. Type IV pit pattern, JNET type 2B - likely high grade dysplasia. Certainly clear planes during ESD

magnification endoscopy used to assess all rectal polyps in addition MRI and high frequency ultrasound prior to decision making. ESD, not full thickness undertaken in this 16x18cm lesion as pre operative endoscopic diagnosis of TVA with high grade dysplasia.

Great team work at King’s Endoscopy. Surgical planning is crucial for resecting massive polyps. 6 hours well spent but challenging in full PPE.

ESD is essential for these large rectal LST G MN. Incidental cancer subjects the patient to more invasive treatment. Propofol for less stressful morning.
