Patient selection is key. RAPID was lower risk than B39 and showed no difference in recurrence. IMPORT LOW serves to me as a gold standard for both recurrence and toxicity:

Agree. With evidence from CREATE-X and KATHARINE trials, going straight to surgery risks missing opportunity to escalate care and improve outcomes (OS and DFS, respectively) for poor responders.
