, as we start to gain experience with MitraClip G4 family, how do we choose the optimal device to treat FMR? If adequate coaptation length (>2mm?) NTW may be preferred, if severe tethering / malcoaptation, PML >9mm, and no concern about stenosis, XTW may be favored.

Close PFO only if patient had dvt. If not I would attribute stroke to AFib and not PFO. Stroke still happens in Afib on AC.
