So in your control file put the "Icon" field that points to that icon in your deb package in an absolute path from the root of the filesystem. So this means that icon also has to be in your deb somewhere.

The Icon URL needs to point to a file inside of of the deb package not to a web link

Package Name? (These probably won't do well with themes unless you have a custom Icon in your control file but I can try anyway)
